Posterior Lumbar Interbody Fusion using Morselized Allograft mixed with Autologous Bone Marrow and Impaction Grafting Technique

Abstract
Purpose: To investigate the efficacy of cancellous allograft with autologous bone marrow and impaction grafting technique for posterior lumbar interbody fusion (PLIF).

Materials and Methods: 31cases were treated with PLIF using cancellous allograft mixed with autologous bone marrow and impaction grafting technique. Among them, 11 cases with allograft for one segment were classified as Group 1 and 20 cases with allograft for two segments was classified as Group 2. 8 cases with PLIF using autologous bone graft were categorized as control group, Group 3. The each groups were analyzed for back pain and clinical results according to Kirkaldy-Willis criteria and Visual analogue scale (VAS) score for back pain as well as radiological fusion rate and changes of disc height.

Results: The clinical results and back pain using VAS were 87.5% and 3.0¡¾2.2, 90.9% and 2.3¡¾1.4, 85% and 2.3¡¾0.8 for the group 1 through 3, respectively. In the study groups, the fusion rate were 81.8% in Group 1 and 70% in Group 2. In the control group, the fusion rate was 100% in Group 3. The allograft group was lessor fusion rate than the autograft group. The changes of disc height were 12.0¡¾3.7 mm and 14.6¡¾3.7 mm and 14¡¾3.6 mm, preoperatively, 16.0¡¾2.3 mm and 17.4¡¾3.4 mm and 17.5¡¾5.0 mm, postoperatively, 13.0¡¾3.2 mm and 15.4¡¾ 2.9 mm and 14.3¡¾3.6 mm, last follow up for the group 1 through 3. The disc height for three groups were increased at postoperative radiographs (P<0.05) and then decreased at final followup radiographs (P<0.05).

Conslusion: PLIF using cancellous allograft mixed with autologous bone marrow and impaction grafting technique would be an alternative method for posterior lumbar interbody fusion, considering comparable clinical results and radiological maintenance of the disc height.
